Submitting New Request

Requests can be submitted for a number of issues

Requests for service can be submitted at any time and will be acted upon by City staff. Once a request has been submitted, you can track the progress of the request using your Ask Lake Forest login. You are required to select a method of contact in which you prefer to be reached.

To submit a new request follow the steps outlined below:

1.      Click the Submit a Request option from the top navigation:

Or select Submit a Request from the Ask Lake Forest home page:

2.      Select the type of request you would like to submit from the Select request menu:

Or you can search for a request type using the Search by Keyword feature:

3.      Once you have selected the request form, you must complete the required fields on the form. If you created an account and are logged into Ask Lake Forest, your contact information will be automatically entered into the appropriate fields for you.

At this stage, you will have the option to attach photos or documents.

4.      Once the form is completed, click the Continue button


5.      Once your request is successfully submitted, you will receive a confirmation screen containing your request ID so that you can track the progress of this request:
