How do I make a public records request? Using the City's Customer Relations Management Program, you may submit a request for a Public Record at any time. Or, you may obtain and complete a Public Records request form on the City's website at http://www.lakeforestca.gov/depts/clerk/records.asp and submit it to the City Clerk's office.

How do I obtain a marriage, birth or death certificate, or divorce decree?
Certified copies of marriage, birth, or death certificates or divorce decrees can be obtained from the County of Orange Clerk-Recorder's Office, located at 630 N. Broadway, Building 12, Room 101, Santa Ana, CA 92741. For general information about and phone-in requests for marriage, birth, or death certificates, please call (714) 834-3005. Certified copies may also be requested online at www.oc.ca.gov/onlineservices.asp.
Copies of City documents are available for a nominal charge at the City Clerk's office at 25550 Commercentre Drive, Suite 100, Lake Forest. The City does not maintain private property records. Please contact the City Clerk at (949) 461-3420 for more information.

City Council meeting agendas can be downloaded from the City's website at http://www.city-lakeforest.com/services/agendas.asp. You can access City Council agendas on the Friday before the City Council meeting.

How do I file a claim against the City?
To file a claim against the City, you must complete and submit a Claims Form. You can download the form or call the City Clerk's Office at (949) 461-3420 to request a claim form. Please leave your name, address and phone number, and a claim form will be mailed to you.
Can I be recognized by the City Council for my Scouting accomplishments?
Yes. Please submit in writing the Scout's name, address and phone number, and reason for recognition, such as Eagle Scout, Girl Scout Gold or Silver Award recipient, and a description of the project to the City Clerk's Office, 25550 Commercentre Drive, Suite 100, Lake Forest, CA 92630.
How do I get City information for a school project?
To have a packet of City information mailed to you for a school project, please contact the City Clerk's Office at (949) 461-3420, and leave your name, address and phone number.
Can I invite the Mayor and City Council to attend an event?
To invite the Mayor and City Council to an event, send an invitation, in writing, to the City of Lake Forest, Attn: City Clerk, 25550 Commercentre Drive, Suite 100, Lake Forest, CA 92630. Please provide three weeks advance notice, if possible.
Can I place an item on the City Council agenda?
No. The City Council does not allow members of the public to place items on the agenda. However, the public is invited to participate in Council meetings by bringing items to the attention of the City Council during the Public Comment portion of each meeting. Although state law prohibits the City Council from taking any action on the item, they may choose to place the item on a future agenda.
