The City of Lake Forest offers door-to-door transportation service to Lake Forest seniors participating in the Senior Scene Clubhouse activities Monday thru Thursday.

To make a reservation for transportation services, please contact Age Well Senior Services, Inc. at (949) 855-9766.  Reservations and cancellations should be made at least 24 hours in advance. The cost is fifty cents per trip.
 
Non-Emergency Medical Transportation

Age Well Senior Services, Inc. offers Lake Forest seniors (at least 60 years of age) non-emergency medical transportation to their medical facilities for $2 one-way, within a designated service area.  The program also serves Kaiser Anaheim (Lakeview), UCI Medical Center in Orange, St. Joseph's Hospital of Orange and the Long Beach Veterans Hospital.  Rides are available Monday through Friday from 7:00AM to 6:00PM.  Participants must be ineligible for the OCTA ACCESS service.  Individuals who are currently utilizing OCTA ACCESS, and are in a frail medical condition, which creates a hardship, may be granted a waiver and be allowed to use the SNEMT Program.

Riders must register with Age Well Senior Services, Inc. before using the service.
